Model-Dependent Realism says that when two models both fit the observable facts, you can choose whichever you like.

You can't have models without consciousness.

"When a model is successful at explaining events, we tend to attribute to it the quality of reality"
You may have misunderstood the above quotation from Mlodinow and Hawking's book. It does not necessarily imply that there is no external reality, only that we tend to believe in our model.
You are conscious. You can choose a model in which the universe would disappear if there were no conscious beings or you can choose a model in which the universe would continue to exist. Both models are consistent with what can be observed.

How can you prove that the universe continues to exist if there are no conscious beings in it to observe it and also, doesn't the concept of infinity evaporate in the same circumstances?

The answers to your questions are obvious and irrelevant.
